Home and Community Based Services Transitional Services, Inc. offers Home and Community Based Services to Medicaid eligible individuals who are enrolled in a HARP Managed Care Plan and are assessed to have need. The list of services offered are: Psychosocial Rehabilitation Services Psychosocial Rehabilitation Services are provided on a 1:1 basis in the individual’s home and are designed to assist the individual with improving functional skills and eliminating interpersonal and/or environmental barriers associated with their mental health or substance use disorder. Activities included are intended to achieve the identified goals or objectives as set forth in the individual’s Recovery Plan. The intent of this service is to restore the individual’s functional level to the fullest possible and as necessary for integration of the individual as an active productive member of his or her family and community with the least amount of ongoing professional intervention. Habilitation Training Services Habilitation Services are typically provided on a 1:1 basis and are designed to assist an individual with a mental health or substance use disorder in acquiring, retaining and improving community living skills he or she never had. Peer Support Services Peer Support Services are peer-delivered 1:1 services with a rehabilitation and recovery focus. They are designed to promote skills for coping with and managing mental health or substance use symptoms while facilitating the utilization of natural resources and the enhancement of recovery-oriented principles (e.g., hope and self-efficacy, and community living skills). Education Support Services Education Support Services are provided 1:1 to assist individuals with mental health or substance use disorders who want to start or return to school or begin formal training with a goal of achieving skills necessary to obtain employment. Pre-vocational Services Pre-vocational Services are 1:1, time-limited services that prepare a participant for paid or unpaid employment. This service specifically provides learning and work experiences non-job-task-specific strengths and soft skills that contribute to employability in competitive work environments. Pre-vocational services can assist individuals to learn a new job and/or maintain a job in a competitive work environment or a self-employment arrangement. The outcome of this pre-vocational activity is documentation of the participant’s stated career objective and plan. Intensive Supported Employment Intensive Supported Employment Services assist individuals with mental health and/or substance use disorders to obtain and maintain competitive employment. Ongoing Supported Employment This service is provided after a participant successfully obtains and becomes oriented to competitive, integrated employment or self-employment. Ongoing Supported Employment is support for an indefinite period as needed by the participant to maintain paid employment. |
